What this tool is and isn't: The UCARE Evaluation tab computes a self-reported wellness index from values you enter yourself. It is not a diagnostic instrument, is not connected to any medical record, and has not been reviewed by a clinician. The PDF it generates attests only that the printed numbers are internally consistent and match a locally computed hash — it does not attest that the underlying values are clinically accurate. Anyone reading the exported receipt should be told this plainly; it is printed on the receipt itself.
